PAYMENT POLICY

Prior to your first visit, our Office Coordinator will confirm your insurance coverage and let you know what your co-pay will be.We accept cash, check, and PayPal payments. We do not accept debit or credit cards, or HSA accounts. All payments are due on the day of the intake appointment. Dr. Leimkuhler is a participating provider with Rhode Island Blue Cross/Blue Shield.

OFFICE POLICIES

Please observe the following policies when visiting our office:

1. Please park your car in a single parking space if one is available and take care not to block an empty space. Do not park in the driveway, as this is a violation of fire codes.

2. No food or beverages are permitted in the building, with the exception of water.

3. Parents are strongly requested not to bring siblings to appointments. Young children are not permitted to stay in the waiting room without an adult, and are not permitted in the consultation room when parents are meeting with the neuropsychologist. Testing and clinical interviews are constantly in session and no noise will be tolerated in the waiting room or outside the building.

4. Parents must escort children under the age of 18 into the building upon arrival in the morning and at lunch time. Please do not drop your child off outside the building. When picking a child up, the parent must come into the building. Children under 18 will not be permitted to wait outside.

5. If someone other than a parent is picking a child up, please inform the psychologist and/or the secretary when the child is dropped off, and give the person's name. He or she will be required to show photo identification when picking the child up.

Directions from Providence and Points North

Take Rt. 95 South to Rt. 4 South (Exit 9).
Rt. 4 will turn into Rt. 1 South.
Go past the exits for Wakefield and Narragansett.
Take the Pond Street exit (not Salt Pond Rd.) off of Rt. 1 South.
At end of ramp, take right onto Pond Street.
Go to the end of Pond Street.
Turn right onto Main Street.
CNLD is the second building on the right (white with blue shutters).

Directions from Westerly

Take Rt. 1 North to the Pond Street exit (after the exit for Post Road).
At end of ramp, take left onto Pond Street. Go to the end of Pond Street.
Turn right onto Main Street.

Directions from I-95 Exit 3

Take Exit 3 onto Rt. 138 East.
Continue for 10-15 min. to Rt. 1, past URI. Turn right onto Rt.1 South.
Go past the exits for Wakefield and Narragansett.
Take the Pond Street exit (not Salt Pond Rd.) off Rt. 1 South.
At end of ramp, take right onto Pond Street.
Go to the end of Pond Street.

Take Exit 5 onto Rt. 102 South.
Stay on Rt. 102 (about 15 min.) to Rt. 4. Take first ramp onto Rt. 4 South.
